problem to add list form in order

hi :-)

i want to add a list form in the form of order, but always throws me a message that say that i have to complete all the fields

i put the list form in "Codigo Postal (ZIP):" for testing
before this, i test putting a new field text and it works well. to do that i modified orders.php, orders_delivery.tpl, orders_print.tpl and the corresponding of admin.

i forgot, i use QC v1.2
