Help Adding Fields to Order Form

I have set up this the Quick Cart and am very pleased with it thus far. I am having one issue. I would like to add a couple of fields to the order form (mainly State and Credit Card Number) and while I can add the fields just fine and add to the proper templates, the form does not seem to pass the entered value.

Some posts on the forum state things like needing to add something to the JS check form script, but I am unsure of how to do that.

If anyone could provide some assistance, it would be greatly apprecaited.

Again, I would like to add a few fields to the order form and pass that information when the form is submitted. Can you help?

add to the oder delivery is right, BUT I need add fields to the ORDER BASKET i need put in that form a combos box for select the time, too i have a function with a calendar, that is called with

escribe_formulario_fecha_vacio("fecha1","fcalen");

it from my actual form, now i'd like do it form QC, help me please!
