"Pleasy fill in all fields" error

I am trying to get QC to work for me, but it never lets me get past entering the order information. As soon as I click on "Send" it says "Pleasy fil in all fields" and redirects me back to customer information page. Can anyone help me? What have I done wrong?

The most common problem here is the Telephone Number format. If you are entering anything other than pure numbers, try it with something like '01234567890' and see if that solves your problem. If it does then you can try other formats.
